Studies on the Protective Effect of Metallothionein Induction Against Nephrotoxicity of Aminoglycoside Antibiotic

Abstract
Gentamicin is an aminoglycoside antibiotic widely used in the treatment of Gram (-) bacteria infections. Both nephrotoxicity and ototoxicity (primarily eighth nerve damage) are prominent side effects of gentamicin.
The present study is designed to confirm the mechanism through which bismuth nitrate reduce the nephrotoxicity of gentamicin.
Bismuth nitrate markedly induced the renal metallothionein. Moreover, the treatment with gentamicin in the presence of pretreatment with bismuth nitrate significantly reduced blood urea nitrogen and urinary glucose content compared with mice of
given
gentamicin alone, which showed the protective effects of bismuth nitrate against the nephrotoxicity of gentamicin. On the other hand, dimethylthiourea (scavenger of hydroxyl radical) did not cause the synergistic action on the protective effects
of
bismuth nitrate. However, the amelioration of gentamicin-induced nephrotoxicity by bismuth nitrate was abolished by pretreatment with indomethacin that is cyclooxygenase ihibitor, which suggests the prostaglandin system is involved in the
ameliorating
action of bismuth nitrate. Therefore, these results suggest that bismuth nitrate can show a protective effect against gentamicin-induced nephrotoxicity via prostaglandin system which may play an important role in ameliorating the nephrotoxicity
of
gentamicin as a defense mechanism of kidney damage.
